	To request an Education Verification, please submit your company's verification form via fax, mail, or email to the Registrar's office and include the following items: 1) Student's full name while attending Twin Oaks High, 2) Student's full date of birth, 3) Year of graduation (if known), and 4) Authorizing signature from the student.
